This one-shot effect is produced by overexposing the moon while shooting with a UV filter. The moon on the right is the real moon, the moon on the left is the ghost produced by the filter.

You didn't reposition the camera mid exposure? I am having trouble figuring out how this worked.
You could do the same shot by using bulb mode doing the first exposure, then covering the camera with a black cloth. After waiting some time for the moon to move through the sky, lifting the cloth and doing the second exposure.
